Another possibility is "the filler out of the death form" misunderstood the
information provided by the informant. Nowhere on the form does the
informant sign the document so they wouldn't know if "the filler out of the
death form" had made a "misprint". I've come across this kind of error
several times so far in the death records.

As you, I'm pretty sure the correct name was GEORGE, not Henwood.
